in-home care

Family, friends and professionals assisting the
caregiver

How do we assist families?
- By offering individualized support,
information, advocacy and referral
- By sponsoring community trainings for
families
- By providing background on important
government programs, legislation and policies
- By supporting the project's advisory
committee that advocates on behalf of older families
- By educating the general community on
the concerns and needs of older families
Goal-to
help families:
- Maintain their quality of life as they age
together
- Develop future care plans for their loved
ones when they are no longer able to provide care
- Connect with other families with similar
concerns
