Info: 45th Street Clinic's mission is
to provide primary health care services and health education, including medical
and dental care, to low income people in North Seattle and North King County who
otherwise could not afford care. It is a non-profit community based clinic that
exist through a combination of public and private funds. The 45th St.
Clinic provides a full range of on-site services including a dental clinic,
social work, counseling, a Class A pharmacy, lab facilities, colposcopy and
other surgical procedures. The provider staff includes five family physicians as
well as physician’s assistants, nurse practitioners and consulting nurses. The patient population at the 45th St. Clinic is
multi-cultural and multi-lingual. Approximately twenty percent of the patients
speak Spanish and many of the providers and support staff are bilingual in
Spanish and English. The clinic serves a large population of homeless clients
through outreach work in shelters and daycare settings as well as evening
clinics for homeless youth twice a week on site.

Info: UW based comprehensive interdisciplinary research and training
center focusing on a wide array of developmental disabilities. CHDD encompasses
two major programs, one focusing on research and the other on clinical services,
training and community outreach, resulting in strong connections between
researchers and clinicians. Provides consultation, training, and research

Info: Community Psychiatric Clinic (CPC) offers a variety of Mental Health and Chemical Dependency Services for individuals enrolled with the State Division of Developmental Disabilities. Services are provided at CPC's Northgate outpatient treatment site. Treatments are individually tailored for those clients who are experiencing or have a history of mental illness and/or chemical dependency. Treatment services may include: case management, psychiatric evaluation, medication management, day treatment and other services. CPC also provides a diversion program for dually diagnosed persons with mental illness and developmental disabilities. This 4-bed, 24-hour hospital diversion program is located at Cascade Hall in the Northgate area, and promotes symptom stabilization and rapid reintegration into the home setting for seriously disabled persons in crisis.

Info: OBCC (which was built during the Civil Rights Era as part of the
federal Model Cities Program,) is an inner-city program of Children's Hospital
and Medical Center. Located in Seattle's Central Area, it has shared a campus
with Carolyn Downs Family Practice Clinic and offices of the Seattle-King County
Health Department since 1994. Together, they form the Central Area Health Care
Center. The Center provides needed services to predominantly low-income,
multi-ethnic families in the surrounding community and offers many special
